Knowing how the region, not just Iran, works, I am not surprised by the response.

There is lots of suspicion of outsiders in many middle eastern countries – bordering on xenophobia.

With such strong feelings and opinions, some people’s thinking gets rigid and some don’t really listen to what is going on.

You can tell them again and again, but they just don’t believe you.

For example, one notion going around years ago was that the U.S. was planning on invading the entire middle east after they took over Iraq.

I kid you not.

After explaining again and again that logistics alone would make that crazy, that there were not enough troops to do it, that it would cost too much, that there just was no strategic interest in some regions and countries to make it worthwhile – it had no effect on my debate opponents.

What can you do, faced with that thinking?

Ignorance, my friend, is the enemy. Everywhere.

Conspiracy fantasies exist even without late night talk-radio?
Or Tea Partiers?
Or Glenn Beck?
Or Birchers?
Or old Nesletters from Ron Paul?
Or anti-Obama birthers, passporters, and other domestic viral opponents of the Democratic administration?

The fact is that conspiracy fears are instigated to frighten and direct the masses, and neither the well-educated Iranians nor the well-educated Americans are fooled by the Rev. Guard words..

But children, the aged, the unlettered among all populations are swayed by well-broadcast fear-campaigns. In Aug., 1945, I was 6 and read of Hitler’s escape to Argentina, along with hundreds of Nazis and tons of gold on secret submarines. Scared many.

Hollywood filmmakers in the Gulf of Oman and faked film of the rescue this week are also implausible but raising them might frighten or convince others. That is why the foreign minister’s remarks hearten all who hope cool heads may someday prevent atomic exchanges or mass bombings in the Middle East.

“The thinking seems to be that if we just bomb her, she’ll start being more reasonable. As I recall, the Japanese had similar notions about Pearl Harbor.”

Your recollection, to say the least, is ludicrous.

The Japanese knew full well that the attack upon Pearl Harbor would awaken the sleeping giant, and that it would incite the vast majority of US citizens to desire war… which it did. They in no way believed that the attack would make the US “reasonably” come to the table to sue for peace.

The attack was intended as a “knock-out” of the Pacific fleet to negate US Navy counteractions to their Imperialistic actions/desires in the entire region. Had they destroyed the US carrier groups, they may well have succeeded.

But don’t let the facts deter you when drawing an analogy.

The US et al, has been at war, unofficially, with Iran since 1979. For good or ill, the likelihood of it soon becoming official, is quite high.

‘For example, one notion going around years ago was that the U.S. was planning on invading the entire middle east after they took over Iraq.

I kid you not. ‘

There was another one too: that we intended to make Iraq into ‘a secular democracy with a tilt towards Israel.’

No foolin.’ And in point of fact, I haven’t noticed considerations of feasibility, realism, or cost interfering much with our decision-making in the Middle East.

We’re perfectly capable of at least attempting to invade the entire Middle East. That it would be a catastrophe for all concerned isn’t an obstacle — nor would it prevent us trying it on.

After all, at the moment we are in all seriousness threatening to attack Iran. The thinking seems to be that if we just bomb her, she’ll start being more reasonable. As I recall, the Japanese had similar notions about Pearl Harbor.

At least we’re still communicating. The majority of people in every country get along and live side-by-side with each other in peace and mutual respect. Iran has every right to pursue nuclear energy. Only transparency is lacking. (I note that even though Saddam offered to allow the US Army to inspect the entire country– even though he himself offered to leave Iraq– Junior Bush was determined to invade in violation of treaty and law.) As long as Israel is caged by enemies, it’s best ideals will be twisted into ferocity and it’s enemies will be persecuted. Jordan thinks itself immune from Palestinians it ejected. Iran cannot believe it will survive a confrontation with US. Talking, not killing, is the smart move. There is much to gain by co-existence, excluding threats. Ahmanidiotjob is desperate to consolidate gains predicated on oppression. Time will overcome his and the mullahs’ tyranny. In every country, we, the people, must prevail.

I haven’t heard anyone in the government ‘crowing’. I’ve heard a lot of well-earned appreciation for the bravery and skill of the US Navy personnel.

This wasn’t simply a sea rescue as a result of a vessel in obvious distress. The Kidd followed up on what they perceived as suspect behavior with a subtle and measured response, saving the lives of the Iranian fishermen and taking the Somali pirates into custody without firing a shot. And all the while respecting the complex political complexities involved in dealing with an Iranian ship.

When professionals are at work, it just looks easy. It isn’t.

As far as the belligerence of the Iranian government, perhaps a simple and sincere ‘Thank you’ should have been the appropriate response?

Both of the Iranian gentlemen quoted are correct. It was indeed a positive humanitarian act. And it was fairly routine if you disregard the rarity of the opportunity. The US Navy is well prepared to conduct such operations at any time. It is their duty to respond to lawless acts at sea and the nationality of the victims is not a consideration. The navies of other nations follow the same protocol.

Our government officials (and the press) should state those facts simply and get on with other business. Crowing about the event should be confined to the story of our good fortune in capturing some pirates in the act.

It would be a good opportunity to kiss and make up, or at least lower the temperature a bit.

…if either side was remotely interested in such a thing. I suspect they’re not. The current regime in Iran must find ‘Iran under siege’ a very convenient device for demanding national unity and excusing all economic failures, while for our part, Obama seems to feel it’s imperative that he demonstrate how tough he can be with gratuitous saber-rattling.

For both parties, this incident is a minor hiccup. They’ll get over it and get back to their mutually beneficial confrontation in no time.

…and if the Republican challenger poses enough of a threat, we’ll even bomb them. And they will retaliate to the best of their ability. And gas will go to $7 a gallon, and most of the world’s economies will tank, and quite a few people will die, and we’ll all feel quite self-righteous.

The crucial point in this narrative is that the Iranian crew had the good sense to tell the pirates that the Americans were going to blow them out of the water — THAT is what led to the quick resolution. If it had been left to the Americans — undoubtedly brave, but hobbled by nonsensical legalistic restrictions in dealing with pirates — we would still be negotiating with the pirates. The Americans acted in properly and in accordance with their rules, but the quick resolution is a result of the Iranian captain’s understanding of how to deal with pirates.

If we could co-exist with the nuclear-armed Soviet Union for a half century without going to war, we ought to be able to co-exist with a half-baked Iran as well. Where is the profit in constantly being at odds with these Middle Eastern nations? Why not a policy of benign neglect? If they attack us (so unlikely as to be almost a complete fantasy), then we should retaliate by the most direct possible means.

But if they don’t attack us, for God’s sake, leave them alone!

This is really a rather trivial story. Naval vessel happens on a fishing boat seized by pirates, rescues fishing boat.

However, that it’s getting so much attention — and that the mildly interesting but essentially unremarkable details are being recited in such exhaustive detail — is perhaps a good sign.

The subtext seems to be, ‘why are we on the verge of war with Iran, anyway?’ It betrays a desire to find a peaceful solution.

Known truthfully only to him and if loyal to one side would NEVER confess the whole truth to the other.

The world grows weary of aggression and particularly USA and IRAN that fight these aggressions on certain principality. It is these principals that Iran dislikes USA. It is on the same principals that USA dislike Iran. Death and destruction, because of misunderstandings and the very means at which death is carried out is a difference !
Hatred is broadened by the governments as giving peace or giving violence as a means to curb the misunderstandings or alienate them….Government in many countries are corrupt and I do not have the power to say which government for I am just a local person praying for peace.
Our world is slowly being destroyed for far worse things than government leadership principals. Our world needs mending, organizing and concentrating on creating solutions to the problems we all have: 1 starvation; 2 famine; 3 over consumption of natural materials; 4 pollution; 5 sharing wealth with less fortunate countries on a broader scale; 6 creation of groups from different countries with commonalities for all, not just a few to reverse this world of material problems and finally; 7 learn about each other and care for each other on a genuine basis. We just do not care for each other because of fear of the unknown !
Is the confessing part of a life long mission to contribute to the hate between USA and Iran, we will never really know.
What are your thoughts !

Poor Hekmati will now have to wait until the US captures an Iranian gamer visiting relatives in the US, so there can be a swap. Unfortunately, the swap may not occur if the detainee happens to be a nuclear scientist too.

Darius Kazemi should probably reconsider plans to visit his grandparents, or else simply meet them in the UAE.

Move aside, Rosetta Stone. The GIs and Marines are evidently getting a lurid sort of linguistic emersion. Perhaps the games depict war as a car chase, complete with road rage epithets, or as a Chuck Norris flick screenplay, where the hero mows down row after row of baddies.

People foget that the US really did try to rescue the hostages in 1979-80. The copters failed, and it was a wild gamble to suppose that the militants would surrender the captives without trouble, or that Tehran’s streets would not become impassible with people or barricades.

If we were colonizing Iraq, we would have took their oil.

Instead, who got the oil contracts from the Iraqi government? A lot of countries, including Chinese.

We weren’t there as colonizers. We went due to some deluded Neocon thinking that we could take out Saddam and then let Iraq be a beacon of democracy that would change the Middle East. Pure fantasy.

As for video games being pro-USA, what the hell do you think it’s going to be? The biggest market for video games is in the US. Do you think a game where you are taliban trying to kill US soldiers and NATO soldiers is going to sell well in New York or California?

The confession sounds like the US to conned some Jar head in to being very stupid. If you do not play video games you have no idea how much US propaganda is imbedded in them. The government has been contracting out for pro US videos games and movies for years. Think what would happen to an Iranian that came to the US to visit his famamil and brought a bunch of anti American games and videos with him. DUH Gitmo. If they could get away with it they would disappear the guy.

All this Iran stuff is just a ploy by the us government to distract US citizens from indefinite detention, the sad state of the economy, and the real threat to the US our own house of representatives.

Iran is not threat to the US never has been. keep in mind that if the US chose to Iran could be turned in to a smoking crater in 45 minutes or less that is not an option because we want their oil.

Our interest in the middle has nothing to do with humanity, democracy, or civil rights. Mineral rights, oil and cheap labor is our goal.

So far the US has been an abysmal failure at colonizing the Middle East.. You cannot enslave sovereign nations in a nice way. The US has to be brutal and scary. that is the way the world works.

If i stand back and try to look at this objectively, i can see this case as a clash of cultures. An American fantasy/propaganda/marketing culture vs an Iranian thought control culture.

In America we are all familiar with Hollywood Fantasy products such as a Rambo rescuing innocent US soldiers from evil North Vietnamese soldiers. Madison avenue products such “An Army of One” to promote military recruitment.

In Iran we are all familiar with the government and religious forces arresting Iranian men and women for cultural infringements on dress and religious codes. I some Islamic countries just converting from Islam is a death sentence. We are all familiar with the death sentences to publishers of cartoons depicting Mohammad.

So it is conceivable that an Iranian court could issue a death sentence to a foreigner who has worked at a company that produces products that denigrate Iran. Just as it is conceivable for America to arrest and imprison Manuel Noriega in his own country for violating US drug laws.

You’re wrong. Banning circumcision does not infringe on religious freedom. It enforces it. Freedom of religion entitles every individual to make their own religious choices. Cutting off part of SOMEONE ELSE’S body is not freedom of religion, it is oppression of religion. The parent’s religion is irrelevant. For true religious freedom, only the boy can make the decision for himself (when he is old enough).

I am a son who was circumcised shortly after birth. I dislike being circumcised so much that I am one of those men restoring my foreskin. The difference is amazing. The more I restore the more I realize how much I lost from having part of my sex organ removed. I will never have everything I lost, but I think every minute restoring has been worth it.

If circumcision is, as one person says, a sign that a descendant of Abraham accepts the agreement between God and Abraham, then let each individual make their own decision about not only their agreement but choosing or not choosing that sign. Infants aren’t agreeing to anything when a part of their natural physical body is taken from them without consent.

Quite clearly, the popularity of circumcision started in the late 1800′s with fallacies and continues with them. It’s mutilation. Six studies say it increases risk of STD’s because it removes immune-system tissue that fight invaders. It decreases physical sensitivity and sexual bonding and promotes a potential for more divorce, which we see in the USA where there is more circumcision than other countries with less rate of divorce.

Female genital mutilation has been federally banned since 1997. That’s part of the push for this movement. If it’s unacceptable for girls even on religious grounds then why the double-standard for boys? Not to mention that FGM ranges from a pinprick to drop one drop of blood to removal of the entire vulva. The extreme versions of FGM were not practiced by as many as the less extreme versions. That certainly doesn’t make it OK, but routine male circumcision in the US is always extreme generally 50% of the skin covering the penis (the lowest end is still more than 30%).

That really can’t be called a sane argument. No study has proven conclusively that circumcision reduces HIV transmission. Even if it did, it wouldn’t be realistic to recommend routine circumcision. The subjects in these studies come from high-risk populations in Africa, yet there is only a very small reduction in transmission. The US is very low-risk. You would have to circumcise HUNDREDS of men to prevent HIV transmission for ONE individual. Everyone would be much safer using condoms a cheap, HIGHLY-effective, PAINLESS way to prevent HIV transmission.

What is so confounding here is the blind eye of people who can see nothing wrong with taking an unconsenting human being, cutting on his genitals and changing him forever. Is that not bizarre, repugnant, perverse, cruel and unacceptable in a civilized world? Let not the rabbis and Jews steer the conversation on the proposed San Francisco ban. They are such a small minority, although the intactivist movement does not and should not leave their victims behind in this. This effort to bring genital integrity to all in America needs to start somewhere. So many men are hopelessly dense on this issue, but there is great hope in mother’s instincts to lead the charge.

How is it possible for someone who never experienced the sexual stimulation of an uncut penis to commentate on the differences between the two? And “[circumcision] promotes a potential of more divorce,” is simply ridiculous. The failure rate of USA marriage has nothing to do with the physical state of a man’s penis. It’s simply a failing of the emotional structure of two people in their relations to each other. There is no possible way you can relate a man’s penis, the part a woman cares the least about in a man, to the rate of divorce because in context the reason two people no longer want to be married is highly emotional. Physicality can come into play with love and emotional ties but the concept of a woman loving a man more if he is uncircumcised is insane, a woman could care less about the state of a man’s penis as long as it is healthy, and health is easier to maintain in a circumcised state. UTI’s can be prevented easier, and STIs are easier to spot and take action against sooner. The entire validity of your argument on this is simply appalling.

“But I think we digress. Are we going to talk about “how much pleasure” circumcision reduces in each sex?”

Of course, wheat and high carbs are THE cause of some people’s diabetes and cardiovascular disease, so that analogy is, how shall I say, poor. We should ban wheat too, if we really care about our fellow’s health.

BOTTOM LINE: A federal ban on female circumcision infringes on the religious and parental rights of immigrants from Africa, Malaysia and Indonesia. Female circumcision is seen as a “religious right” and a “parental decision” in these families too, but nobody seems to be concerned about that. The fact that circumcision advocates have to turn the focus from “religious freedom” and “parental choice” to “which one is worse” tells us one thing: The “religious freedom” and “parental choice” arguments are rubbish.

“These analogies do not impress the rabbinical community of San Francisco. Rabbi Joshua Strulowitz of the modern Orthodox shul Adath Israel says that this is just one of many efforts throughout history — as recently as Soviet times — to prevent Jews from practicing brit milah, or the covenant of the foreskin. As for the ‘intact’ argument, he points to the Talmud, which says that, just as God made wheat so that humans could perfect it into bread, circumcision is a way of completing God’s work. “We are partners with God in creation,” he says.”

The benefit is actually quite small. A 3% reduction in the probability or something. When you consider that you really should be wearing a condom anyway, its not an especially good way of lowering STD rates.

Female rights has always been about how women “feel” about a particular issue. The cutting of the rose may be a religious practice but it’s not common enough that people are comfortable with it. It all has to do with how common something is. Circumcision is common, because it’s been done for thousands of years and on such a widespread level that people have become accustomed to it. There are advantages medically to prevent infection and such but as with any medical practice that’s been around for years obviously people are going to object.

good point: Banning circumcision absolutely infringes religious freedom. For a true believer of the Jewish and (I would assume) Islamic faith, banning this practice goes directly against the word of god, and not circumcising means depriving one’s son the right to participation in one’s tradition and spiritual practice. When that tradition views sexual pleasure as secondary to eternal spiritual fulfillment, you’d better believe a law banning this fundamental rite, believed to be commanded by God, looks draconian.

Well, what about the sane arguement that circumcision helps prevent the spread of AIDS, as well as helps to stop cervical cancer in women?

I did ask Schofield if he had any financial involvement with, for example, the ‘restoration’ business. He said he had none, for what it’s worth. I tend to believe him: Intactivists seem to just view this as a rights issue.

Marty, you seem to imply ADHD is not a real disease, and if you are you would be correct. It’s not a disease. It-like its name implys-is a deficintcy. A real problem that has real data and research. Would you not call bi-polar disorder or deppresion a real medical issue? Obviously you’re making a ignorant comment, i doubt you have ADHD and I doubt you’ve experiance going through school with it, having children with it, or anything assosiated with it. ADHD medication corrects the rell ADHDers deficentcy in things like attention and hyperactivity, allowing them to function close to the level they would of a person with no deficentcy (not that it works for all people, or very effectivly alone). Think of it like this: Would you deney medication to a person suffering from depression just becuase others abuse it? Would you deney car crash victums pain killers becuase theres a chance it would be sold on the street? All medicines are going to be abused by someone, ADHD medicine is being singaled out at this moment becuase of its ingrediants. But I doubt anyone will live to see the day cough syrup will be taken off the shelf becuase of its unbeatable level of abuse and easy acsess.

Lalala, I really think you need to speak with people who have recovered from ADD and medication use.

I am a neuropsychiatrist by training and since the birth of the ADHD syndrome, that pretty rapidly turned into a pandemic disease (particularly in the US), I have tried to understand how this had possibly happened. I am not questionning the existence of this disease but wondering how it could affect so many, initially children and now adults being diagnosed with it as well. My explaination is that it is the result of a our almost unescapable overstimulating environment that works on people like addictive drugs: they need more and more stimulation or they get ‘bored’ (I wonder what this is because I have never experienced boredness). From a practical perspective, what I would suggest to parents is 1. let children express their imagination by giving them pro-toys (pots, wooden spoon, blocs) rather than expensive manufactured toys and let them work around the house with you 2. do not teach them alphabet (they are not monkeys) but give them paper, crayons and let them express themselves as they wish 3. rather than using a computer, let them use (with supervision) your camera, play the keyboard (if you have one), garden with you…. 4. let them engage in physical activities. On the other hand, we need to make the schools children friendly (promoting trust and relaxing control), with activities that fit their nature (being children), this means many more breaks between classes. In any case, the response to the pandemic ADHD is certainly not amphetamines. I do agree that their overuse is very concerning. What we probably do not know yet is what are the long term outcomes of amphetamines chronic use, do they increase cardiovascular risk? I hope that teachers will stop suggesting to the parents of challenging kids that their child may have ADHD and should get a prescription.

I think there is not as much “wiggle room” in education to allow children with ADHD to be average students or even poor students. I honestly would not allow my children to take this medication if they were able to learn without it. My kids have always been late bloomers. I was aware of their issues before kindergarten so they each spent an extra year in preschool. They both had extra tutoring and in class support. Now a year into medication our nine year old is on track and our eight year old is a bit behind. The very first day my daughter took Concerta she went to her reading tutor and was able to finish a book. She said “Mom a whole new world is opened up”. I can’t imagine the psychological damage that she would suffer if her adhd wasn’t treated.

Prior tochild. In reality there are many more uncommon symptoms that ADHDers have, and a badly behaved child is not one of them.

Worried about this myself-I have a 1st grader who most certainly meets the DSM criteria for ADHD. The school almost forced an IEP and drugs on us, but we resisted and changed his diet and got a little improvement. I just hate to see him put on meds. He is gifted and I am afraid those stimulants will damage his wonderful little brain. If we can just stand him that long.

It’s up to the drug companies to allocate the ingredients between brand and generic drugs. So, not surprisingly the generic drugs were the first to go. I haven’t been able to get generic methylphenidate for two months in Seattle. While the active ingredients may be the same, the drugs affect you differently. It is a huge pain it the a.. to switch medications after you have gone through the process of trying different drugs to find one that works without side effects. The generic worked great for me, and the brand names have not been as good.

Could it be that there are simply more generic ADHD medicines available now (Adderall XR became generic this year) and/or more people are taking generic ADHD medicines because they can’t afford brand name drugs and someone didn’t predict the supply/demand correctly?

The shortage of ADHD “medicine” is really a symptom of how many people have been fraudulently diagnosed with this “disease.”

This terrifies me. I have two children with ADHD. We tried many things before we used concerta. If this drug becomes unavailable to my kids it will be impossible for them to lead productive lives.

(d. 1804) (r. 1777–1804) warlord and governor of Acre Cezzar Ahmed Pasha was a Bosnian adventurer who became the dominant political personality in Syria in the last quarter of the 18th century. His greatest triumph was in 1799 when, from his fortress in the town of Acre, he stalled the advance of Napoleon Bonaparte. Ahmed left his native Bosnia and, like many other young Muslim men, sought his fortune in Egypt, where wealth and opportunity abounded. There he gained employment as a hired gun in the household of Bulutkapan Ali Bey in the 1760s. In that role, he dealt ruthlessly with the Bedouins of the Libyan Desert who were in rebellion over taxes imposed upon them by his master Ali Bey. It was they who gave him his nickname, Cezzar (the Butcher), for the execution of dozens of Bedouin tribesmen.

In the political vacuum created after the death in 1775 of Zahir al-Umar, the chieftain of the Ziyadina clan who had dominated what is today northern Israel for almost half a century, Cezzar Ahmed sought to ingratiate himself to Sultan Abdülhamid I (r. 1774–1789) by reasserting thesultan’s authority in the port city of Acre. He was rewarded in 1777 with the governorship of the Lebanese province of Sidon, a position he held until his death. Although the authorities in Istanbul feared that he aimed to establish his autonomy from the empire, as his former masterin Egypt had done, Cezzar Ahmed was able to secure the governorship of Damascus at several different times, with his longest tenure in that office lasting from 1790 until 1795. Through his abilities to defeat the rebellious Druze and Shii clans of southern Lebanon, and by providing local security and enabling the uninterrupted flow of taxes to Istanbul, Cezzar Ahmed simply made himself too valuable for Istanbul to remove. Nonetheless, the Ottoman government remained wary and tried to remove him from power whenever possible. Sensing his vulnerability, Cezzar Ahmed chose to remain in heavily fortified Acre even when he held the governorship of Damascus, and did not take up permanent residence in that city.

In a century when locally prominent families, the ayan, dominated the politics of Syria, Cezzar Ahmed was a consummate outsider, being a non-Arab without a large extended family to support him. Having lived in Egypt and risen through the ranks in a Mamluk household there, he sought to replicate a similar patron-client relationship in Acre with his own mamluks, who remained subordinate to him and under his direct command, whilesometimes holding governorships in Tyre and Tripoli.

Learning from the mistakes of Zahir al-Umar, Cezzar Ahmed was careful to keep himself in good graces with the sultan and his court. His two main rivals for control of southern Syria were the al-Azm family, whose members frequently filled the governorship of Damascus, and the Shihab family in the Lebanese mountains. Although the two families often worked together against his rising political power, Cezzar Ahmed usually held the upper hand due to his wealth and his phalanx of hired men.

Cezzar Ahmed’s moment in the international spotlight came in 1799 when he was able to withstand the siege of Acre by Napoleon Bonaparte, albeit aided by an outbreak of plague that decimated the French ranks. Napoleon’s forces had quickly dispatched the Mamluks in Egypt the year before, and the French general had assumed that the Ottoman Empire would offer no stiffer resistance. But Cezzar Ahmed’s stubborn refusal to surrender, added to the harassment his forces were suffering from the British fleet in the eastern Mediterranean, forced Napoleon to reconsider his plans for empire in the Middle East, and he returned to Egypt. Napoleon left Egypt not long after his less than triumphant campaign in Syria, but his forces stayed on to occupy the country until 1801 when a combined British-Ottoman force secured their surrender.

A career in a professional association can be an extremely rewarding way to practice clinical pharmacy while serving the members of the profession. Positions may focus on many different programmatic areas (Table 1). In addition to positions in national associations, pharmacists serve important roles in state and regional associations. There are also many related biomedical associations in which pharmacists may be employed.

TYPES OF POSITIONS AVAILABLE

Many types of positions are possible within associations. Those working for large pharmacy associations may have more than 100 coworkers. Employees of large associations typically have fairly narrow responsibilities and focus. Within smaller associations, with staffs of 40 or less, areas of responsibility are broad, and many positions are combined or do not exist.

Approximately 20 of the 100 employees of the American Pharmaceutical Association are pharmacists. Of 20 employees of the Academy of Managed Care Pharmacy, 4 are pharmacists. The American College of Clinical Pharmacy, with a staff of 30, employs 6 who are pharmacists. The American Society of Health-System Pharmacists, with a staff of approximately 175 persons, has a larger proportion who are pharmacists due to their extensive publishing efforts.

Clinical pharmacists within associations are most frequently employed within areas where their backgrounds are of most benefit. Some of these areas include serving as the chief executive officer, working with chapter relations, education, government and professional affairs, marketing, membership, and publications, including the journals of the association.

Depending on the size of the association staff, clinical pharmacists may start out working with specific publications, membership and chapter activities, or educational program planning. As they gain experience and familiarity with their roles, they may take on greater managerial roles and broader areas of responsibility. Those with the greatest experience and expertise may move into director, or vice presidential, positions, or even become the chief executive officer for the association.

According to the American Society of Association Executives (ASAE), the average total compensation for association executives in 1996 ranged from $48,000 to $117,000. Compensation has increased over the last few years by percentages greater than business averages (5-10% for many positions). Many associations use ASAE survey information, faculty and deans’ salary data collected by the American Associations of Colleges of Pharmacy, and salary data from other segments of the profession (e.g., managed care pharmacists, industry pharmacists) as benchmarks for pharmacist executive staff salaries.

Regardless of the association’s scope or focus, pharmacists must be responsive to the needs and desires of the membership served. Each association employee is ultimately responsible to the members and prospective members for whom the association exists. The membership at large is generally represented by an elected governing board (Board of Directors, Board of Regents, Board of Trustees), and sometimes a house of delegates. Typically, the activities of the association are guided through a strategic planning process, which sets the direction for the association for the next few years.

CAREER PATHS AND PREPARATION

Several paths exist for entry into a career with a professional association. Many association executives have had an earlier career as a practicing member of their profession. They have worked “in the trenches” for a number of years, and were active in the association as a volunteer, through committee work, attendance at meetings, presentation of scientific abstracts, or elected office. By becoming known as a reliable volunteer, they set the stage for movement into association employment. This path is advantageous in providing the association with an employee whom they know and who understands the membership and the profession. However, a disadvantage of this path is that clinical pharmacists have rarely received the management training and experience necessary for association work.

Others who identify association work as their career objective early in their training may enter this type of work through summer internships, experiential clerkships and externships, and executive residencies, or by accepting their first postgraduate job with an association and then advancing though various positions and departments. Many of these opportunities are listed in a document prepared by the member organizations of the Interorga-nizational Council on Student Affairs.

Regardless of the career path chosen, preparation for a career in association management should ideally include several elements. Exposure to association work can be obtained through an internship, a clerkship, or an executive residency. These experiential activities provide excellent opportunities for exposure to what it might be like to work for an association.

Experience as a practicing member of the profession can be an excellent way to prepare for association work. Through spending time in actual patient care, research, and teaching, prospective association pharmacists can learn firsthand what members do and what their struggles are. Furthermore, by becoming involved with associations on a volunteer basis, it is possible to experience how the association works. Volunteering also provides an opportunity to showcase an applicant’s talents and dependability to the association. Choosing to work for employers who will allow time and support for association work is also beneficial.

Other areas of preparation that are very helpful involve skills aside from those taught in traditional pharmacy curricula. Those thinking of careers in associations should keep their eyes open to issues and events, the actions of other related groups, and the opinions of colleagues. They need to pay attention to the business and the politics of the profession and be informed about them.

Finally, training in how to manage projects, people, and finances can be very helpful for association managers. Many of the skills necessary to be an excellent clinical pharmacist are opposite those required to be an excellent manager. Training and experience in this area are crucial to excelling in an association career.

BENEFITS OF ASSOCIATION WORK

There are many benefits of association work. Working within an association provides increased opportunities to be involved with the discipline of clinical pharmacy. For someone who has practiced patient care, conducted research, or taught, this can provide an exciting new direction with new challenges and new skills to learn. Those who have been members of the association before working for it often appreciate the opportunity to work for a group having a mission they believe in and from which they have benefited as a member.

Association work necessarily involves communication with its members. Those working in this field have many more acquaintances among their peers and with representatives of the pharmaceutical industry. Establishing and maintaining contacts is an important part of this work. Those who enjoy interacting with people will especially enjoy this aspect of association life.

A clinical career within an association can be an opportunity for continued learning and professional growth. Association managers often need to enhance their skills in business management, public relations, marketing, project management, advocacy, politics, personnel management, legal issues, and many other areas, depending on the scope of their responsibilities. Adopting the attitude of a lifelong learner is essential, as the profession and the world change rapidly. It is necessary to constantly learn new knowledge and skills to keep up with these rapid changes.

Working for an association opens many career opportunities in related fields, with other associations, with the pharmaceutical industry, in other management jobs, and in private consulting. The skills gained through association work are widely transferable to other professions, as well as back to clinical practice.

Clinical pharmacists within associations have many opportunities to work on new projects pertaining to the membership and the profession. They can be creative not only in developing a project, but also in funding, implementing, and promoting it. Association work provides a wide variety of opportunities to practice clinical pharmacy in ways that, although different from traditional roles, affect patient care and the profession.

Many of the knowledge and skills taught in clinical pharmacy curricula are applicable to association work. For example, those working with educational programs often draw on their pharmacy backgrounds to help members design better educational offerings. Also, research techniques learned in academic training are applicable to association research.

Perhaps the greatest benefit of association work is the ability to represent the association’s members, seeking to improve their practices and opportunities, and to make a difference in the profession.

CHALLENGES OF ASSOCIATION WORK

Those choosing to practice clinically within a professional association must have excellent organizational and time management skills. They will experience many pressures from various directions and must be skilled at prioritizing their activities. Excellent written and verbal communications skills are also essential.

Association executives must find a balance between being a member of the association and serving on the staff. An attitude of servanthood is absolutely essential. It is also critical to find a balance between listening to members and responding to their needs versus taking the lead and establishing the direction of the association. The most responsive associations are typically “member driven,” meaning that the association staff take their lead from the elected officers of the organization, with a focus on implementing, rather than establishing policy.

When working with speakers, authors, and committees, who by definition are performing volunteer work, association executives must be prepared to help members meet their deadlines, through reminders, clear communication, and advance planning. Missed deadlines by members have the potential to result in periods of great stress for association staff who in turn have printing or other deadlines that cannot be changed. As this can be a source of great stress, it is essential to have the personal fortitude to deal with the unexpected and uncontrollable.

Because associations must function as businesses, serving as stewards of their members’ resources, there is less freedom for association employees to function as independently as they might in an academic environment. Budgets must be developed and adhered to, and performance targets established and met. Also, because the elected officers change regularly, the direction of the association and the style of the governing board can change frequently. Clinical pharmacists working within associations must be flexible, as issues and responses can change, may not always agree with their personal opinions, and are often outside their control.

CONCLUSION

Working as a pharmacist within a professional association is a challenging, yet rewarding way to practice clinical pharmacy. This career choice should be strongly considered by those who seek to serve their colleagues and the profession by helping them manage and adapt to ever-changing issues in clinical pharmacy.
